Set-Up Account
1. Put in Username that was given to you.
2. Put in Password that was given to you.
3. Click LOG IN
4. Click Get My Custom Plan Now! Located on top left of page.
5. Click on Complete Waiver
6. Read the waiver.
7. Click Yes (You read the waiver).
8. Click Yes (You agree to the waiver).
9. Click Submit.
10. Click Complete PARQ.
11. Answer all questions.
12. Click Submit.
If you answered YES to one or more of the questions we require a Medical Release.
13. Click on Complete Medical Release.
14. Click Yes if you wish to continue without your Doctor’s written approval.
15. If you wish to talk to your Doctor before continuing:
Click Download Medical Release and Print.
Click I will fax my Medical Release.
Note: You will not be able to generate a workout until we receive the faxed Medical Release from your Doctor.
16. Click on Complete Intake Questionnaire.
17. Answer all questions and click Submit. (You will be able to Update this as often as you like.)
18. Click on Complete Self Assessment.
19. Answer all questions and click Submit. (You will be able to Update this as often as you like.) It will tell you what level you are in all four areas.
20. Click Complete Workout Schedule.
21. Click on the days you want to do which exercises and click Submit.
22. Your workout will be generated if you have all Green Check Marks by the needed information..
23. If you want to print it, click Printer Friendly and then PRINT from your computer.
